The National Mail Voter Registration Form can be used to register U.S. citizens to vote, to update registration information due to a change of name, make a change of address or to register with a political party.

Fill Out the Form
Fill out the 1-page application.
Note: Any US citizen residing in the fifty states can use the form, with the exception of those residing in North Dakota, Wyoming and the US territories.
STEP 3: Mail In the Form
Mail in the form to your state or location election office for processing. Be sure you mail it in an envelope with the proper amount of postage . You'll receive a confirmation from the office that you are registered.
